Plug in a code reader and look for codes. When they don't start almost immediately it is either ignition or fuel injection. However, I am certain that you have a check engine light and it is throwing a few codes. Cold starting problems are the fault of the cold start circuitry which enriches the overall air fuel mixture. Hot or any other starting condition is the air fuel mixture. You can test it simply by using carb spray and giving it 2 seconds of carb spray, then cranking. If it starts immediately then it is your fuel injection. Since this is all computer driven, you have to look for codes and fix the bad codes.......

I would not suspect the FPR because it does not cut off fuel, only has a window of regulation. Since it starts and runs, it really can't even be the regulator....

You could not be more WRONG on that statement.

A leaking FPR will cause excess fuel in the vacuum lines, causing a very rich mixture at first. It will start, but run like crap until the excess is gone.

I have just begun noticing these hard start symptoms in my 99 Silverado. Looks like a new FPR is in my near future......

2001 8.1 could be the crank position sensor there was a recall on them and they can cause hard starting problems and can also cause the engine to stall, check the fuel pressure while running and also while turned off. check for spark while cranking need to figure out weather it is spark or fuel related. to check the regulator you can take the vacuum line off and see if any fuel comes out where the vacuum line attaches.

Be sure to check for gas in the vacumn line after just shutting the engine down as sitting over night the gas will drain back out of that line and give the appearance the fpr is ok. I recently went through this with my 2000 GMC. I posted what I did in another forum. Here's the link.

father in laws truck shut off on our way hunting today. Cranks and wont start. I changed filter and no fix. auto store suggested fuel pressure regulator....Where the heck is it located.2001,4.3 silverado 2 wheel. Thanks

Wait! I just saw the link with a pic previous to my question.MY truck does not have a fuel rail that the pic in the link does but that is the exact FPR they gave me. My fuel lines go into the rear of the intake,??? SO WHERE WOULD My REGULATOR BE.did they give me the wrong one??

Does it start hard after truck has sat over night or a long period of time or does it just start hard after you've drove and engine is warm and has been shut off for 15-20min?

Pump pressure checked with the proper tool?
My 2000 van had hard start problems from a bad rotor(high resistance between the spring steel and the thicker bar).
In addition, a short draining the amps out of the battery has also been a problem. 13 volts work well. 12.5volts did not. I attached a Sears trickle charger up to the battery and had a red trouble light come up at first. The following morning the trouble light was off, full charge was on and the van started just fine. My instrument gauges come back to life 10 seconds after removing the key from the ignition.

Invest in the right tool to check the pressure. It is worth the investment. you will use it for the rest of your life.

And I forgot to mention that the coolant temp sensor measuring more than 5 degrees difference from the air temp sensor when cold can be a problem.
